Prescription Items

Please tell us which drugs you require. Details can be found on the right hand side of your repeat prescription. You can either:

Select the "Please issue all my Repeat Medications" option at the top, using the "Special Medication Request" box at the bottom to expand if you need to, eg "I don't need my amlodipine", or "I'm ordering early since I am going on holiday for 2 weeks 1st June" or
Enter the name of each drug, along with the strength and dose, in the boxes below (If you require more than 15 items, please complete and submit another request form)
Lastly, select your preferred collection option - collect from the surgery or choose one of our local chemists from the drop down menu.
